Adult Plan (18 and older)

- Includes 2 routine exams and 2 cleanings
- All necessary X-rays (Pano and CBCT)
- 2 fluoride treatments
- 1 urgent care exam with X-rays
- Cosmetic consultation
- 2 oral cancer screenings
- 15% off all treatments
Child Plan (through age 17)
$405/year
- Includes 2 routine exams and 2 cleanings
- All necessary X-rays
- 2 fluoride treatments
- 1 urgent care exam with X-rays
- Hygiene instructions
- 40% off sealants
- 15% off all treatments
Add-On: Periodontal Maintenance Plan
$215/year
- Includes 2 additional periodontal maintenance appointments
- 2 extra fluoride treatments
- Laser pocket treatments during periodontal therapy
- 15% off scaling and root planing
Plan Disclaimer:
In-house coverage must be paid in full and will expire on the day before the one-year anniversary date. If the Additional Perio plan is needed as an add-on, the coverage will expire on the same date as the existing adult or child plan, no matter the sign-up date. The price shall remain the same. *New patients or existing patients who decide to sign up for the in-house insurance within 30 days of their last preventative or diagnostic appointment can use their payment toward the plan sign-up cost.
